Navigator
Navigator is a program funded by state government that aims to support disengaged learners aged 12 to 17 years of age and work with them around a return to education or alternative pathway.
The program works closely with families and schools and uses assertive outreach, case management and restorative practice to re-engage children back into learning pathways.
Jesuit Social Services is the provider in the Brimbank Melton, Hume Moreland, North Eastern Melbourne and Inner Eastern Melbourne areas, working alongside local schools, services and the regional Department of Education and Training office.
Connexions
The Connexions program was founded in 1996, in response to the identified needs of marginalised young people experiencing high and complex needs.
It seeks to engage young people in relationships of trust and understanding, providing a professional service of flexible therapeutic counselling, clinical assessment, assertive outreach, case management, suicide prevention, drug and alcohol interventions, clinical consultancy, advocacy, and information and referral.
The Mental Health Clinician will work closely with the Navigator Program Coordinators, to engage with and support the mental health needs of young people on the program, across Hume Moreland, Brimbank Melton and North Eastern Melbourne Navigator regions.
Additionally the role is embedded in the Connexions team that will provide specialist support to each of the Navigator teams and program more broadly.
The Mental Health Clinician role will be integrated into the Navigator program model that consists of Active Hold Support (phone based), Targeted Case Management, long term Case Management and Mental Health specialist support through our Connexions Program.
This is a full time role and will working across both our Collingwood and Sunshine offices.
