We are looking to recruit 2 enthusiastic and experienced Case Managers who are able to provide case management to men and women exiting prison.
ReConnect provides transitional support to eligible participants who are exiting custody and will be residing in the North and West Metropolitan regions of Melbourne.
In order to facilitate community reintegration and reduce reoffending the program supports participants to address seven key domains; housing, mental health, AOD, employment, education, training, independent living skill, family and community connectedness.
These 2 part time roles (0.8 EFT and 0.9 EFT) are located at Brunswick, with the requirement to travel between prison locations.
